In this form, you may submit for the request types listed below:

Know & Data Portability: You may request that we disclose certain information to you about our collection and use of your PI.
Deletion: You may ask us to delete your PI. We may deny your request if certain statutory exemptions apply.
Correction: You may ask us to correct inaccurate information about you. We may deny your request if the contested information is more likely than not accurate.
Appeal a Denial: You may appeal a privacy rights request that was previously denied by us.
Additional State Privacy Rights: Any additional privacy rights provided under your states’ data privacy laws.
